Cancellation Policy

The office requires 24-hour notice for cancellations of all appointments.

No Show/ No Call before 24hrs to appointment will be a $50 fee

We accept cash, Visa, Mastercard, Discover, American Express, and personal checks.

Co-payments are expected during your visit.

If you are a self-pay patient or if you are subjected to a deductible that has not been met, payment is expected at the time of service.

If you have an insurance change please bring your new insurance card to your visit.

We encourage patients to keep a credit card on file to make the checkout process easier, faster, and more efficient.

Self-Pay Established Patients Total Charge $180 per visit.
